There are five steps to joining our network. Please review each section to learn more about these steps and what to expect after you apply.
Important Notice: New Enrollment Process for Carelon Behavioral Health
Providers wishing to join our network as Individuals, Groups, or as a provider joining a group must use Availity for enrollment.
Please note the following important details: -
- Enrollment Through Availity: All new enrollment requests must be submitted via the Carelon Behavioral Health Payer Space in Availity.
- Registration Required: Providers must be registered with Availity to utilize this new enrollment process. If you have not yet registered, please visit www.Availity.com to complete your registration.

Applicants interested in joining Carelon’s Network may submit a request to join. Submitting your information and the required documentation does not guarantee inclusion in Carelon’s Network(s).
California Providers: If you wish to be considered for enrollment in Medi-Cal, you will be required to submit a Medicaid ID or proof of application through DHCS Provider Application and Validation for Enrollment (PAVE).
Effective April 1, 2025, Carelon Behavioral Health will no longer accept new Medicaid provider applications for Northeast Health Partners or Health Colorado, Inc. For further information, please email Coproviderrelations@carelon.com. Existing in-network providers with Northeast Health Partners or Health Colorado, Inc. who wish to add practitioners to their in-network agreement may continue with the enrollment process.
If you are currently an out of network provider with a member needing care, please refer to our OON guide for processing.
New provider, currently participating practices and provider group:
To request to join our network, please enroll using the Carelon Behavioral Health payer space by visiting Availity.com. Providers must be registered with Availity. If you have not yet registered, please visit www.Availity.com to get registered.
New facility:
Request to join the network as a new facility by completing our Request to join the network. Please return the completed form, including the Terms and Conditions, via email to bh_incoming_agreements@carelon.com.

Incomplete, incorrect, or illegible forms may delay or prevent proper processing. If you have any questions, call our National Provider Services Line at 800-397-1630 Monday to Friday, 8 a.m. to 8 p.m. Eastern time.
New provider:
Once you've completed and submitted your request for participation, and we’ve confirmed you meet eligibility requirements, we’ll send the contract through our secure application to the email you used to register.
Please review and sign your contract.
New facility:
Once you've completed and submitted your request for participation, and we’ve confirmed you meet eligibility requirements, one of our contract associates will contact you.
Currently participating practices:
If you’re a provider joining a group with a participation agreement, you will not receive a contract directly. Instead, you'll be added to the existing agreement once you've been approved for credentialing.
Our credentialing process fully complies with national accreditation standards and applicable state and federal laws, rules, and regulations. Our credentialing process ensures you’re part of a quality network where in-network referrals can be trusted.
You must be credentialed and approved before being added to the network. To remain in the network, you must follow a re-credentialing approval process every three years or the period required by state laws or regulations.
Council for Affordable Quality Healthcare (CAQH) participating providers:
To eliminate the need to submit multiple credentialing applications, we participate in the CAQH.
If you’re registered with CAQH, please add Carelon Behavioral Health as one of the organizations authorized to access your information and also ensure your attestation is current. If you’re not registered with CAQH, visit CAQH.org to set up an account and complete an application.

Non-CAQH participating providers:
If you're not a CAQH-participating provider, we encourage you to contact our National Provider Services Line at 800-397-1630 for further instruction.
Facility:
Contact our National Provider Services Line at 800-397-1630 for further instruction or to check the status of your credentialing request.
Please note that you must be approved through the credentialing process and obtain a countersigned contract before seeing patients as an approved network provider.
To check your status, please use the secure link provided in the email you received during enrollment.
Throughout the credentialing process, practitioners have the following rights:
- To Review information submitted to support their credentialing application. The organization is not required to make available, references, recommendations or peer to peer protected information.
- The right to correct erroneous information. Please refer to the provider manual upon approval to join the network for further information. When information is received by a third party that conflicts with the information submitted by the provider. The discrepancy must be correct in writing within seven (7) calendar days of notification and submit any corrections directly to the credentialing staff’s attention.
- The right to access the status of your application. To view the current status, log in to Availity.com and select "submitted applications”.
Please note, if you have submitted your application on or after May 19, 2025, you can check your application status by visiting Availity.com .
If you meet eligibility and contracting requirements to join our network, you will receive formal communication from us with your effective date.

Submit W-9 Form
W-9 Forms are required for Out-of-Network Providers. Failure to provide the required W-9 Form may result in claim denial. Please fax the required W-9 Form to Carelon Behavioral Health at 866-612-7795.
